Case Management
Our Men’s 28-Day Residential Treatment Program provides a supportive, highly structured recovery environment that respects the unique culture of each participant and builds a solid foundation for long-term recovery.
Upon admission, each adult participant completes a medical and biopsychosocial assessment to formulate an individualized patient care plan for ongoing success. In addition to addressing the symptoms of substance use disorders, we prioritize each participant’s overall health and well-being. Through the process, residents participate in group, individual, and family therapy as well as life skills education.
The Transitional Housing Program (THP) equips adult men experiencing homelessness to achieve long-term recovery and well-being. Through this program, residents develop and practice skills needed for employment by engaging in education, training, and substance use treatment.
Each participant is supported by a team leader, recovery coach, housing specialist, and peer mentor. Our unique Peer Leader group is directed by alumni who understand the challenges participants face. Alongside our knowledgeable and caring staff, participants build housing and recovery plans and participate in community initiatives that focus on advocacy and empowerment.
