Case Management

Our Men’s 28-Day Residential Treatment Program provides a supportive, highly structured recovery environment that respects the unique culture of each participant and builds a solid foundation for long-term recovery. 

Upon admission, each adult participant completes a medical and bio-psychosocial assessment to formulate a custom treatment plan for ongoing success. In addition to the symptoms of addiction, we prioritize each participant’s overall health and well-being. Through the process, residents participate in group, individual, and family therapy as well as life skills education.

The Transitional Housing Program (THP) equips homeless adult males to achieve long-term recovery and well-being. Through this program, residents develop and practice skills needed for employment by engaging in education, training, and addiction treatment. 

Each participant is supported by a team leader, recovery coach, housing specialist, and peer mentor. Our unique Peer Leader group is directed by alumni who understand the challenges participants face. Alongside our knowledgeable and caring staff, participants build housing and recovery plans and participate in community initiatives that focus on advocacy and empowerment.

The Permanent Supportive Housing (PSH) program provides community-based housing assistance and supportive services to stabilize households consisting of at least one member with a recent or current history of substance use.

Operating under the Housing First model, we prioritize immediate access to housing, ensuring no preconditions or program completion requirements are necessary. While we offer a full range of supportive services to all participants, engagement in these services is voluntary—securing and maintaining housing stability does not depend on mandatory participation.
