Family Case Management

The Family Case Management Program is designed for eligible pregnant women or women with infants less than one year of age who meet the Department of Human Services Community Health and Prevention income requirements.

The program ensures that clients have access to proper medical care, with the goal to decrease infant mortality, and to promote health for the family as a whole.

In addition, a wide variety of other social services are provided to support other aspects of their lives. Case managers are assigned to clients to identify their needs and to develop plans to meet those needs.

The program assists clients with their needs in obtaining primary care physicians for themselves and/or their child, nutritional services, health education, transportation, mental health and substance abuse treatment.
