JOB DESCRIPTION

JOB TITLE: Addiction Counselor

SUPERVISOR: Program Manager

ESSENTIAL FUNCTIONS:

Under the supervision of the Program Manager, the incumbent provides individual and group                counseling to participants, which includes presenting educational lectures, leading process groups, formulating and reviewing individualized treatment plans, and maintaining progress notes.  The incumbent must be able to communicate effectively with participants.

QUALIFICATIONS:

A.        Master’s degree in the Social or Human Sciences of Social Work or a minimum of two      (2) years in college working on degree is preferred.

B.        Must be certified by IODAPCA or certifiable within two (2) years from date of hire.

C.        Must have a thorough knowledge of substance abuse and its modalities of treatment.

D.        Must have knowledge of ASAM criteria and DMS-IV diagnosis.

E.         Must have knowledge of DHS regulations (2060/2090) and CARF.

F.         Must have at least two (2) years of counseling experience in the substance abuse and         MISA (mental health) certified.

G.        Knowledge of Illinois DUI laws.

H.        Bilingual/Bicultural (English/Spanish) preferred.

I.          Knowledge of MS Office Applications.

J.          Flexible to adapt to schedules and work load to the needs and demands of the program.

K.        Possession of a valid Illinois driver’s license and willingness to use personal vehicle in       course of employment.

RESPONSIBILITIES:

1.         Lead process groups.

2.         Responsible for an assigned caseload and in establishing and reviewing individualized      treatment plans.

3.         Provide an initial individual orientation for each new participant.

4.         Provide individual sessions to participants as needed.

5.         Complete documentation of participants’ progress in the clinical records on a daily basis.

6.         Contact participant’s case worker/referral source within the first week of treatment.

7.         Provide each participant with written treatment verification, if requested.

8.         Provide referral sources to participants when needed.

9.         Present cases at clinical staffing.

10.       Maintain participant records in accordance with agency standards.

11.       Comply with agency policies and procedures.

12.       Participate in regular agency staff meetings and training.

13.       Participate in agency fundraising efforts.

14.       Perform other duties as assigned by supervisor.
